Pharmacological effect

The CP is active against various microorganisms, including aerobes (staphylococci, streptococci, gonococci, moraxella, hemophilic rod and others), anaerobes (bacteroides, propionibacteria) and atypical pathogens (chlamydia, mycoplasma, ureaplasma). A wide range of actions favorably distinguishes it from the obsolete Erythromycin; low resistance of target microorganisms - from Azithromycin. Josamycin does not concede to it on properties, but it is an expensive antibiotic of a reserve.

Clarithromycin penetrates well into various body fluids and accumulates there in amounts exceeding the plasma content. To such liquids can be attributed the fluid of the inner and middle ear, which is separated inside the sinuses of the sinuses. This property of Clacid promotes its wide application in ENT-practice.

Indications for use

Treatment of infectious-inflammatory diseases caused by pathogens sensitive to clarithromycin:

  • odontogenic infections;
  • eradication of Helicobacter pylori in patients with duodenal ulcer or stomach ulcer (only in combination therapy).
  • infection of the upper respiratory tract and ENT organs (tonsillopharyngitis, otitis media, acute sinusitis);
  • infections of the skin and soft tissues; mycobacterial infections (M.avium complex, M. kansasii, M.marinum, M. leprae) and their prophylaxis in AIDS patients;
  • infection of the lower respiratory tract (exacerbation of chronic bronchitis, acute bronchitis, community-acquired bacterial and atypical pneumonia);

Contraindications

You can not use an antibiotic for treatment if a person has such diseases and conditions:

  • high sensitivity to the components of tablets and other drugs from the group of macrolides;
  • renal failure in severe form;
  • lactose intolerance, glucose-galactose malabsorption, lactose deficiency;
  • porphyria;
  • age to 18 years.

It is not allowed simultaneous administration of the antibiotic Klacid SR and the following drugs: Pimozide, Astemizol, Terfenadine, cisapride, Dihydroergotamine, Ergothamine, as well as with oral forms of medications midazolam, Alprazolam, Triazolam.

The drug is administered with caution in case of impaired functioning of the kidneys, liver, or myasthenia gravis. Care must be taken when taking the medications that metabolize in the liver at the same time.

Application in the first trimester of pregnancy is contraindicated.

Application in the II and III trimesters of pregnancy is possible only in cases where the intended benefit to the mother exceeds the potential risk to the fetus.

If necessary, use during lactation should stop breastfeeding.

Instructions for use

In the instructions for use, it is indicated that CP Clatidae are long-acting tablets that are intended for adults and adolescents over 12 years of age, provided that their body weight is not less than 40 kg. If the teenager has reached 12 years old, but his body weight is below 40 kg, then he should be given Clacid exclusively in the form of a suspension.

Tablets are optimally taken with food, swallowing whole, without cracking, not chewing or grinding in other ways, but with a small amount of still water.

Dosages of Clacidum SR are the same for adolescents and adults and depend solely on the severity of the course of the infectious-inflammatory process. For infections of any localization of moderate or mild degree of severity, Clacidum is recommended to take 500 mg (1 tablet) 1 time per day. In severe infections, the dosage is doubled and taken at 1000 mg (2 tablets) once a day.

People suffering from kidney failure with a creatinine clearance less than 30 ml / min should not take CP Clathid, because it is impossible to accurately dose it. This category of people should take Klacid in tablets of 250 mg or a suspension. If people suffer from kidney failure with a creatinine clearance of more than 30 ml / min, they can take Clacidum SR at half the dosage, that is, 250-500 mg once a day.

Side effects

Post-marketing research data are more extensive and include the following undesirable reactions:

  1. Cardiovascular and hematopoiesis system can respond to this antibiotic by changing the heart rate (ventricular tachycardia) and blood composition. In the latter case, thrombocyto- and leukopenia are observed.
  2. There may be a transient decrease in hearing and taste perversions. The latter are often accompanied by changes in the sense of smell.
  3. Allergic reactions, sometimes caused by oral administration of the drug, range from minor skin rashes to anaphylaxis and epidermal necrolysis.
  4. From the side of the nervous system, according to several reports of taking the medicine, not only painful sensations in the head, but also sleep disturbances, increased anxiety, confusion, the appearance of nightmarish dreams. However, there are no data confirming such effects, and the listed symptoms are most likely, far-fetched (especially nightmares at night).
  5. Violation of the balance of natural microflora, characteristic for antibiotic therapy with drugs of any group, can lead to the reproduction of yeast-like fungi and, accordingly, candidiasis.
  6. Hepatic dysfunction of varying severity (up to cholestatic hepatitis), but reversible. Several deaths are attributable not so much to the use of klatsida, as to the combination of it with other drugs against a background of liver failure and concomitant serious pathologies. There are reports of pancreatitis.

However, as already mentioned, side effects are quite rare and even in people with immunodeficiency conditions, which take this antibiotic for months, appear in light form.

Overdose

Existing reports indicate that an overdose of clarithromycin can cause symptoms on the part of the gastrointestinal tract. One patient with a bipolar psychosis in history who took 8 grams of clarithromycin developed a violation of mental status, paranoid behavior, hypokalemia and hypoxemia.

Adverse reactions accompanying an overdose should be treated with gastric lavage and symptomatic therapy. As with other macrolides, hemodialysis or peritoneal dialysis is unlikely to significantly alter the level of clarithromycin in serum.

special instructions

  1. It is possible to develop cross-resistance to clarithromycin and other antibiotics of the macrolide group, as well as clindamycin and lincomycin.
  2. Patients with chronic liver disease require regular monitoring of serum enzymes.
  3. During therapy, it is possible to develop pseudomembranous colitis from mild to life-threatening severity. CP clapidosis can cause changes in normal intestinal microflora and, as a consequence, growth of Clostridium difficile. C caused. difflcile pseudomembranous colitis should be suspected in all cases with the appearance of diarrhea after the use of antibacterial drugs. After the completion of the course, careful medical supervision of the patients' condition is shown. There are data on the development of pseudomembranous colitis 2 months after the end of the drug.
  4. Infections of soft / mild to moderate skin are mostly caused by Staphylococcus aureus and Streptococcus pyogenes. In this case, they can show resistance to macrolides, in connection with which it is necessary to carry out a sensitivity test.
  5. Continued use of Clacidum CP can cause colonies with an increased number of insensitive fungi and bacteria. When superinfection requires the appointment of appropriate therapy.
  6. There are reports of the onset of hepatic dysfunction, which can be severe, but in most cases reversible, in some cases - with a fatal outcome, which is mainly due to the presence of serious concomitant diseases and (or) combination therapy. When itching, anorexia, jaundice, darkening of the urine, tenderness of the abdomen during palpation and other symptoms of hepatitis, Clacid SR is immediately withdrawn.

When acute hypersensitivity reactions appear, treatment with Clacidum SR is immediately canceled and appropriate therapy begins.

Drug Interactions

The tablet pills have a drug interaction, which is important to consider when designing a complex treatment regimen. Below are some of the pharmacological limitations, reflected in the instructions for use, voiced by the attending physician:

  1. If a patient who regularly receives drugs primarily metabolized with CYP3A isoenzymes is prescribed a CP Clacid, there may be an increase in side effects, a decrease in protein concentration.
  2. Antibiotic drug Klatsid SR 500 in combination with Quinidine or Dizopyramid can provoke pirouette tachycardia.
  3. Clarithromycin increases the concentration of HMG-CoA reductase inhibitors, enhances the effect of oral anticoagulants.
  4. In combination with carbamazepine or Teofillin, the concentration of the latter increases in the systemic blood flow.
  5. In combination with Tadalafil, Tolterodinom, Clarithromycin, Vardenafil or Sildenafil, a dose of Clatidum SR 500 should be reduced, otherwise the risk of side effects increases.
  6. When interacting with Itraconazole, Atazanavir, Oxacillin, and Saquinavir, bi-directional effects of Clarithromycin are observed.
  7. In a complex with Verapamil and Clarithromycin, there are frequent attacks of arterial hypotension, bradyarrhythmias.
    Clutch SR 500 enhances the action of colchicine in renal failure, and poisoning of the organism is excluded.
  8. In a complex with Triazolam, there are nervous disorders, for example, drowsiness and confusion.
  9. The combination of an antibiotic with quinidine or dizopyramide provides a gradual development of hypoglycemia.
  10. The medical preparation stimulates the activity of Digoxin, while systematic monitoring of the level of digoxin in the blood plasma is required.
  11. In order not to disrupt the therapeutic effect of this medication, it is necessary to perform an individual dose adjustment following medicines: Rifabutin, Efavirenz, Nevirapine, Rifapentin, Rifampicin, Fluconazole, Ritonavir.

Reviews

We picked up some of the people's feedback on the Clacid SR preparation:

  1. Svetlana. A good antibiotic Klatsid was assigned to me by a doctor with sinusitis, when the illness so acute that I was already advised to make a puncture, but I insisted on the treatment with pills. It's clear the doctor avenged me and appointed Klatsid - who saw how much it costs, he will understand me. Truth despite the high price, having spent a week's course I felt relief and I did not need a puncture. Then she found other Clarithromycin analogues cheaper, produced by Russia.
  2. Hope. The reaction of his daughter to him is terrible. even fast called: the diarrhea is the strongest and the pulse goes off scale. And the doctor from Medsi was in a course of a tachycardia, any nystatin has not written out.
  3. Dasha. I was appointed after pneumonia. Having taken the first pill, I immediately felt an unreasonable sense of fear, then nightmares began to dream. And after she woke up was still very long under the impression. Plus, there was a very unpleasant sensation in the abdomen, a feeling as if I was being inflated from the inside. In short a nightmare... It remains to drink another 7 tablets, but I doubt that I will continue... I'll ask the doctor for an alternative.

Reviews about Klatsida SR differ. Many note its effectiveness, rapid action and relief of symptoms. However, in other cases, there is a lack of effect and development of pronounced side effects, including digestive system disorders, headache, panic attacks. Also from the shortcomings is the high cost of the drug.
