Pharmacological effect

It is an antipsychotic agent - a neuroleptic, derived from phenothiazine. The drug has antiemetic, anti-allergic, sedative, hypothermic. The neuroleptic effect of the drug is due to the blockade of dopamine receptors by the mesocortical and mesolimbic system. The sedative property of the drug is due to adrenoreceptor blockade in the brainstem. Clinically, the effect of the drug is expressed in terms of an angry, as well as angry-irritable kind of affect. At the same time, the decrease in aggression is not accompanied by the effect of inhibition and lethargy.

Antiemetic effect of the drug is due to blockade of dopamine receptors of the hypothalamus. It inhibits the release of mediators, which reduces the permeability of membranes of presynaptic membranes, and also disrupts their deposition and re-uptake. Antiallergic effect is due to blockade of histamine receptors peripheral. The hypotensive effect is caused by blockade of adrenergic structures. The drug is effective in cases of behavioral disorders, especially in children, and also facilitates the possibility of contact with patients.

Indications for use

Medication is prescribed under the following conditions:

  • acute psychotic disorders;
  • anxiety;
  • psychomotor agitation;
  • chronic psychotic disorders such as schizophrenia;
  • chronic non-schizophrenic delusional disorders: paranoid delusional disorders, chronic hallucinatory psychoses (for the treatment and prevention of relapses);
  • aggressive or dangerous impulsive behavior (as an additional drug for short-term treatment of these conditions).

Contraindications

Absolute:

  • Parkinson's disease;
  • Retention of urine due to diseases of the prostate;
  • Closed-angle glaucoma;
  • Porphyria, agranulocytosis in the anamnesis;
  • Simultaneous application of levodopa;
  • Lactation;
  • Hypersensitivity to the drug.

Relative (use neuleptil follows with extreme caution in connection with the risk of complications):

  • Renal / hepatic impairment;
  • Diseases of the cardiovascular system;
  • Elderly age.

Use in pregnancy and lactation

Although the use of the drug during pregnancy is not prohibited in some cases with prolonged therapeutic treatment Neuleptil in women there were extrapyramidal and gastrointestinal disorders, for example, bloating and others. Therefore, first of all, it is expedient to find out whether the expected benefit from taking the drug exceeds the possible harm.

In addition, it is recommended to shorten the duration of treatment with the drug during pregnancy. If medical necessity requires the use of the drug in the third trimester, it is recommended to observe some time behind the nervous system of the newborn.

Due to the lack of reliable data on the effect of the drug on breast milk, Neuleptil is not recommended for taking during lactation.

Dosage and route of administration

The instructions for use indicate that the dosage regimen Neuleptil varies significantly depending on the indications and age of the patient. The average daily dose should be given in 2 or 3 admission, with an emphasis on the evening hours.

  • In adults, the average daily dose can range from 30 mg to 100 mg. In some cases it is permissible to increase the daily dose to 200 mg.
  • In children older than 3 years, the average daily dose is from 0.1 mg to 0.5 mg per kg of body weight.

Side effects

The drug is usually well tolerated, but sometimes unwanted reactions are possible. Their severity varies depending on the pharmacological properties of Neuleptil. Small initial doses of the drug may trigger the following side effects in a vegetative nervous system: dry mouth, urinary retention, constipation, orthostatic hypotension, paresis of accommodation.

In some cases, the central nervous system may be disturbed: drowsiness or sedation (maximum expression at the beginning of therapy), anxious and depressed condition, apathy, mood changes.

Higher doses of Neuleptil are capable of causing functional disorders from the nervous system: tardive dyskinesia, early dyskinesia (trisus, oculomotor crises, spasmodic torticollis, etc.), extrapyramidal disorders (motor agitation, akinesia, sometimes associated with increased muscle tonus and partially eliminated with anticholinergic antiparkinsonian drugs, akathisia, hyperkinesia-hypertonus).

There is a possibility of development of metabolic and endocrine disorders: frigidity, impotence, hyperprolactinemia (gynecomastia, galactorrhea, amenorrhea), weight gain, hyperglycemia, thermoregulatory disorders, decreased glucose tolerance.

The following side effects appear less frequently and do not depend on the dose of the drug:

  1. Hematologic disorders: rarely - leukopenia, agranulocytosis (careful monitoring of the general blood test is required).
  2. The organs of vision: a decrease in the tone of the eyeballs, formation of brownish deposits in the anterior chamber of the eye that do not affect vision (as a result of accumulation of the drug).
  3. Skin reactions: photosensitization, allergic skin manifestations (rashes, itching, redness, etc.).
  4. Other: risk of development of cholestatic jaundice, positive serological test for antinuclear antibodies (without symptoms of lupus erythematosis).
  5. Malignant neuroleptic syndrome. If an unexplained fever occurs, antipsychotic treatment should be canceled, as this may be one of the signs of a malignant neuroleptic syndrome (manifested by hyperthermia, pallor of the skin, dysfunction of the autonomic nervous system). Predisposing factors for the occurrence of this effect are organic lesions or dehydration of the brain.

Overdose

If the drug is used in a dosage exceeding the therapeutic norm, extrapyramidal disorders in severe form may appear.

In the case of an overdose, treatment is performed aimed at suppressing undesirable symptoms. In some situations, therapy should be carried out in specialized departments.

special instructions

Before you start using the drug, read the special instructions:

  1. It is not recommended to take alcohol during treatment.
  2. For patients with epilepsy, due to the ability of the drug to lower the threshold of excitability of the cerebral cortex, clinical and, if possible, electroencephalographic observation should be performed.
  3. In case of fever or infection, a general blood test should be performed, as there have been reports of the potential for agranulocytosis.
  4. Neuroleptics of the phenothiazine series may contribute to the prolongation of the QT interval, which increases the risk of serious ventricular arrhythmias such as torsade de point, which are potentially dangerous (sudden death).
  5. The prolongation of the QT interval is particularly enhanced with bradycardia, hypokalemia and congenital or acquired (as a result of taking medication) QT interval prolongation. Before prescribing therapy with neuroleptics as an absolutely necessary treatment factor, and if the clinical picture allows, To exclude the possible emergence of risk factors, medical and laboratory tests must be carried out.

Care should be taken with pericyazine:

  • in elderly patients, due to their high predisposition to the development of sedation and orthostatic hypotension;
  • in patients with severe cardiovascular pathology, due to hemodynamic disorders and changes in the ECG;
  • in patients with hepatic and / or renal insufficiency, due to the risk of overdose.

Drug Interactions

It is strictly forbidden to use this drug together with Levodopa, since it has been established that there is a mutual antagonism between these two drugs. The effectiveness of Neuleptil is significantly reduced with alcohol consumption, and also with concomitant administration of guanethidine, sultopride and other drugs related to antihypertensive drugs and affecting the fibrillation of the ventricles.

With great caution, one should treat joint use of Neuleptil and hypotensive drugs, drugs that affect the nervous system, as well as antacids, holinoblokatorov, including atropine.

Reviews

We picked up some reviews of people who used the drug Neuleptil:

  1. Inna. My son literally saved this drug, stopped the development of a very bad disease. The son went to college and studied. The main thing is to choose the right dosage and then gradually reduce it. Be healthy!
  2. Olga. My friends give "Neuleptil" to a teenage son suffering from schizophrenia (in an aggressive form). Before that, tried many other drugs, but "Neuleptil" was the best. First, this tool really corrects behavior, and not just slows down. That is, after taking the medicine the child can quietly do the lessons, engage in some kind of hobby, and not just befuddled sitting on the couch. Secondly, the remedy does not cause fast addiction. The boy takes it for six months, until everything is in order. Other medications stopped functioning after 3 months. Disadvantages of "Neuleptil": unpleasant taste, weight gain.
