Corinfar is a medicinal product that has the ability to selectively block calcium channels and is widely used in cardiac practice.

The main action - causes antianginal and hypotensive effects, promotes relaxation of the smooth muscles of blood vessels. Corinfar cures spasm and dilates coronary and peripheral arteries, has a beneficial effect on blood circulation, contributes to a decrease in peripheral resistance. Also, the active substance helps to reduce the need for myocardium in oxygen, reduces OPSS and blood pressure.

Among other effects - a slight diuretic effect, blocking the effect on platelet aggregation.

Clinical and pharmacological group

The calcium channel blocker.

Pharmacological effect

The active substance of Corinphar, nifedipine, is a synthetic derivative of dihydropinidine and belongs to the group of calcium channel blockers.

The therapeutic effect of taking the drug comes after 20 minutes and lasts for 12 hours. When administered orally, the preparation is well absorbed from the digestive tract, the maximum level of plasma concentration is observed after 1.3-4 hours after administration. Metabolized completely in the liver and excreted from the body by the kidneys in the form of inactive metabolites, 20% is excreted with bile.

The mechanism of action of the drug is due to the ability of nifedipine to block slow calcium L-type channels, slowing the penetration of calcium ions into the smooth muscle layer of the heart and vessels. Due to a decrease in the level of concentration in the myocardium of calcium ions, the contractile activity of vascular wall cells, the expansion of coronary and peripheral vessels decreases. Corinfar increases coronary blood flow, activates the work of collaterals, improves blood flow to the myocardium in ischemic zones. The use of the preparation reduces the OPSS by expanding the peripheral arteries, increases the tone of the myocardium, while reducing its need for oxygen.

The use of Corinfar for a long time, more than 3 months, causes tolerance of the organism to its action.

Indications for use

According to the instructions for the use of Corinfar and the prolonged form of Corinfar retard, the drug is used to reduce pressure and therapy in the following conditions and diseases:

  1. IHD (ischemic disease).

The dose of the drug is selected by the doctor individually in accordance with the severity of the disease and the sensitivity of the patient to the drug. For patients with concomitant severe cerebrovascular disease and in elderly patients, the dose of the drug should be reduced.

Simultaneous food intake delays, but does not reduce the absorption of nifedipine from the gastrointestinal tract.

Chronic stable and vasospastic angina

The initial dose is 10 mg (1 tab.) 2-3 times / day. With an insufficiently pronounced clinical effect, the dose of the drug is gradually increased to 2 tab. (20 mg) 1-2 times / day. The maximum daily dose is 40 mg (4 tablets / day).

Essential hypertension

The average daily dose is 10 mg (1 tab.) 2-3 times / day.

With an insufficiently pronounced clinical effect, a gradual increase in the dose of the drug to 20 mg (2 tablets) is possible 2 times / day. The maximum daily dose is 40 mg (4 tablets / day).

With a 2-fold appointment, the minimum interval between doses of the drug should be at least 4 hours.

special instructions

In the period of receiving Corinfar, observe the following rules:

  1. The appointment of the drug in conjunction with adrenoblockers is possible, but the reception should take place under strict medical supervision.
  2. The drug is not compatible with alcohol. For the entire period of treatment from drinking alcohol should be discarded.
  3. If a person has severe heart failure, then calculate the dosage should be especially careful.
  4. The drug can give a false positive result of a direct Coombs reaction, tests for antinuclear antibodies.
  5. The drug is able to influence the activity of spermatozoa during in vitro fertilization.

  7. In a patient with severe obstructive cardiomyopathy, the frequency of seizures, their severity and duration may increase. In this case, it is recommended to cancel taking the drug.

Taking the drug, you need to be extremely careful in the process of driving vehicles. From performing actions that require increased attention, it is better to refuse for a while (consider this if your work requires concentration).

Drug Interactions

When applying Corinfar with other drugs simultaneously, the possible reactions of interaction should be considered:

  • Nitrates - increased tachycardia;
  • Diltiazem - suppression of the metabolism of nifedipine;
  • Rifampicin - acceleration of nifedipine metabolism;
  • Prazosin and other alpha-adrenoblockers - suppression of their metabolism, increased hypotensive effect;
  • Vincristine - a slowing of its excretion and, as a consequence, an increase in side effects;
  • Amiodarone, quinidine, disopyramide, flekainamide - strengthening inotropic action;
  • Quinidine - a decrease in its concentration in the blood plasma;
  • Cephalosporins, digoxin, theophylline - an increase in their concentration in the blood;
  • Lithium preparations - intensification of toxic effects (diarrhea, nausea, vomiting, ataxia, tinnitus);
  • Procainamide, quinidine and other drugs that cause prolongation of the QT interval - the risk of a significant prolongation of the QT interval;
  • Nonsteroidal anti-inflammatory drugs (NVS), sympathomimetics, estrogens - reduction of hypotensive effect;
  • Inhibitors of the CYP3A isoenzyme, including antiviral drugs (eg, nelfinavir, indinavir, ritonavir, saquinavir, amprenavir), antifungal azoles (fluconazole, itraconazole, ketoconazole), valproic acid, macrolides - an increase in the concentration of nifedipine in the blood plasma;
  • Preparations with a high degree of binding to proteins, including indirect anticoagulants - derivatives of indandion and coumarin, NSAIDs, salicylates, anticonvulsants, quinine, sulfinpyrazone - increasing their concentration in the blood plasma;

  • Carbamazepine, phenobarbital - a decrease in the concentration of nifedipine in blood plasma.

During the period of coronary treatment, grapefruit juice should not be consumed, because it suppresses the metabolism of nifedipine.

Reviews

We picked up some reviews of people taking the drug Corinfar:

  1. Vladimir. Corinfar is an ambulance at high pressure, half a table under the tongue and pressure will decrease after 15 minutes.
  2. Stas. The doctor prescribed these pills for hypertension, took once, the pressure really decreased, drank at night at a pressure of 140 to 110, in the morning there was a pressure of 94 to 74 too low. I'll try to take a pill, maybe it will not fall so low.
  3. Anna. When I had problems with pressure, the doctor prescribed Corinfar pills. I was satisfied with their price, we did not find any contraindications. Taking these pills even in the early days, I immediately felt better, the pressure indicators became more stable. At first, there was a slight nausea, but I can not even say exactly what the drug was.
  4. Love. I was diagnosed with hypertrophic cardiomyopathy rather late, when the disease was already on its own know in the form of sharp attacks, increased pressure, loss of consciousness, clouding in the eyes and noise in the ears. The disease is accompanied by angina and arrhythmia. How many tablets have not tried, do not always help. I've heard a lot about Corinfar, and almost everyone in one voice said that the drug really helps. I also began taking Corinfar, and the pressure more or less stabilized. But all the same till now I am in search of more complex or difficult acting medicine.
