The main element of the drug is a substance such as monetazone furoate, which has an anti-inflammatory effect and eliminates manifestations of an allergic nature.

The popularity of the spray for sinusitis and other complications of an allergic rhinitis is explained by its high efficiency and mild action combined with good tolerability and practical absence of serious side effects.

  • Active substance: mometasone furoate (micronized, in the form of monohydrate) - 0.05 mg;
  • instagram viewer
  • Auxiliary components: benzalkonium chloride (as a 50% solution), citric acid monohydrate, purified water, polysorbate 80, sodium citrate dihydrate, glycerol, microcrystalline cellulose, carmellose treated with sodium (dispersed cellulose).

Pharmacological effect

Nazoneks - SCS of local action. Has anti-allergic and anti-inflammatory effect, reduces the production of inflammatory mediators. Prevents the accumulation of neutrophils, which reduces the amount of inflammatory exudate, reduces the processes of infiltration and granulation.

Systemic bioavailability of Nazonex is low, the active substance is not detected in the plasma even when sensitive methods of detection are used. The minimal doses of mometasone can enter the digestive tract and are excreted with the help of the liver.

Indications for use

According to the instructions for use, the following indications are given for the use of Nazonex:

  • treatment of seasonal and year-round rhinitis in adults, children older than two years;
  • treatment of adenoids in children;
  • seasonal exacerbation of sinusitis (complex antibiotic treatment is used) in adults, including senile age, and children over 12 years of age;
  • prevention of seasonal allergic rhinitis, which occurs in the middle and severe stages (2-4 weeks before the dusting season).

Contraindications

Contraindications to the appointment of Nazonex are:

  • intolerance to any of its components;
  • active or latent tuberculosis infection of the respiratory tract;
  • the presence of an untreated / untreated local infection, provided that the mucous nasal cavity is involved in the process;
  • an untreated bacterial, systemic viral or mycotic infection, as well as an infection caused by the herpes simplex virus with eye damage (in some cases, the drug can be prescribed by way of exception, as directed by the treating physician).

If a patient in the recent past suffered a nasal injury or a nose operation, the use of the spray is contraindicated until the wound is healed.

Use in pregnancy and lactation

Nazonex during pregnancy is not contraindicated for prescription, but is used extremely rarely due to the lack of statistically reliable data on its safety for the health of the mother and fetus.

Provided that the doctor nevertheless recommends treatment with Nazonex to a woman during pregnancy (a preparation with a genyantritis of an allergic nature is usually difficult something to replace), after childbirth it is necessary to comprehensively survey, in order to exclude congenital hypofunction of adrenals.

If allergic rhinitis is seasonal in nature, it is not recommended to prescribe for prophylactic purposes: for this purpose it is better to choose hormone-deprived medicine from the impressive arsenal of now existing means. The same applies to mothers breastfeeding.

Dosage and route of administration

The instructions for use indicate that Nazonex is intended for intranasal use.

1 dose = 1 injection and contains 50 μg mometasone.

Treatment of seasonal or all-the-year-round allergic rhinitis:

  • Children 2-11 years of age: the recommended therapeutic dose is 1 injection each nostril once a day. Young children with the introduction of the drug required the help of adults.
  • Adolescents from 12 years and adults, (incl. elderly people): recommended therapeutic and prophylactic dose - 2 injections in each nasal passage 1 time per day, after reaching the necessary therapeutic effect for maintenance therapy, the dose can be reduced to 1 injection in each nostril 1 time per day. If it is not possible to achieve a reduction in symptoms, you can increase the daily dose to 4 injections of Nazonex in each nasal passage. After the condition is improved, the dose should be reduced.

The onset of the action of NAZONEX normally occurs 12 hours after the administration of the first dose.

In acute sinusitis and exacerbation of chronic sinusitis, adults and adolescents from 12 years of age usually have 2 injections in each nasal passage 2 times a day. If the improvement does not occur, it is possible to increase the daily dose to 4 injections in each nostril 2 times a day. After reducing the severity of symptoms, the dose should be reduced.

For the treatment of acute rhinosinusitis (in the absence of signs of severe bacterial infection) in adolescents from 12 years and adults, Nazolex requires 2 doses per nasal passage 2 times a day. If the condition worsens, you should consult your doctor.

For polyposis, adults, including elderly patients, are prescribed 2 injections per nostril 2 times a day. Once the severity of the symptoms of the disease decreases, it is recommended to reduce the dose to 2 injections in each nasal passage 1 time per day.

Rules for the use of the drug Nazonex

The introduction of the spray is carried out using a special dispensing nozzle on the bottle.

  1. Before the first application of the Nazonex nasal spray, it is necessary to calibrate by pressing the dosing device 10 times, until there are splashes, indicating that the preparation is ready for use.
  2. You should tilt your head and inject the drug into each nostril as recommended by your doctor.
  3. If the nasal spray is not used for 14 days or longer, you must press the dosing nozzle 2 times until splashes appear.
  4. Before each use, vigorously shake the bottle.

To avoid malfunctioning of the dispensing nozzle, it must be cleaned regularly as follows:

  1. Remove the protective cap first, then the spray tip.
  2. Rinse them in warm water and rinse well under running water.
  3. Dry in a warm place.
  4. Attach the tip to the bottle.
  5. Screw the protective cap.

When using Nazonex for the first time after cleaning the nozzle, you need to calibrate - press the metering nozzle 2 times.

Do not try to open the nasal applicator with a needle or other sharp object, because this will damage the applicator, resulting in an incorrect dose of the drug.

Side effects

The most likely side effects are:

  • sneezing (short-term);
  • irritation of the mucous membrane of the nasopharynx;
  • headache;
  • nosebleeds of medium intensity, which quickly pass and do not require medical care;
  • inflammation of the mucous membrane - pharyngitis (occurs with prolonged use of the drug);
  • burning, itching, unpleasant pain in the nasal passages (appear immediately after instillation, when the substance gets on the mucosa, and for a short time pass).

Unlikely side effects:

  • pain in muscles and joints;
  • the appearance of cataracts or glaucoma;
  • increased intraocular pressure;
  • fast fatigue, lethargy, depression;
  • dysfunction of the adrenal glands;
  • Cushing's syndrome (chronic overabundance of glucocorticoids, which is manifested by weight gain, changes in skin condition, excessive hairiness in women, changes in the psyche and other symptoms).

Overdose

Due to the fact that the systemic bioavailability of the drug Nazonex is <1% (using the quantitative determination method with a threshold of sensitivity (LLOQ) of 0.25 pg / ml), it is unlikely that an overdose will require other measures, except for monitoring the condition of the patient with subsequent use of the drug at the recommended dose.

special instructions

As with any long-term treatment, patients using Nazonex nasal spray for several months and longer, should periodically undergo examination by a doctor for possible changes in the nasal mucosa. It is necessary to monitor patients receiving intranasal GCS for a long time. Possible development of growth retardation in children. In case of detection of growth retardation in children, it is necessary to reduce the intranasal SCS dose to the lowest, which allows to effectively control the symptoms. In addition, you should refer the patient to a consultation with the pediatrician.

When conducting placebo-controlled clinical trials in children, when nasonex nasal spray was applied at a daily dose of 100 mcg during the year, there was no growth retardation in children.

In case of development of a local fungal infection of the nose or pharynx, it may be necessary to stop therapy with Nazonex nasal spray and conduct special treatment. The long-lasting irritation of the nasal and pharyngeal mucosa can also serve as a basis for discontinuing Nazolex nasal spray treatment.

With the use of intranasal GCS, it is possible to develop systemic side effects, especially with prolonged use in high doses. The probability of these effects is much lower than when using oral GCS. Systemic side effects can vary both in individual patients and depending on the GCS used. Potential systemic effects include Cushing's syndrome, characteristic cushingoid symptoms, suppression of adrenal function, growth retardation in children and adolescents, cataract, glaucoma and less often a number of psychological or behavioral effects, including psychomotor hyperactivity, sleep disturbance, anxiety, depression or aggression (especially in children).

With prolonged treatment with Nasonex nasal spray, no signs of suppression of the hypothalamic-pituitary-adrenal system function were observed. Patients who switch to Nazonex nasal spray treatment after long-term therapy with systemic SCS require special attention. The abolition of SCS systemic action in such patients can lead to adrenal insufficiency, the subsequent recovery of which can take up to several months. If signs of adrenal insufficiency appear, the systemic GCS should be resumed and other necessary measures taken.

Patients undergoing GCS treatment have potentially decreased immune reactivity and should be warned about the increased risk of infection in case of contact with patients with some infectious diseases (for example, chicken pox, measles), as well as the need for medical advice if such a contact has occurred. When there are signs of severe bacterial infection (eg, fever, persistent and sharp pain on one side of the face or toothache, swelling in the orbital or periorbital area) immediate medical advice is required.

During the transition from treatment of SCS systemic action to treatment with Nazonex nasal spray, some patients may experience initial withdrawal symptoms of systemic GCS (eg, joint and / or muscle pain, fatigue and depression), despite a decrease in the severity of symptoms associated with nasal mucosa. Such patients should be specially convinced of the advisability of continuing treatment with Nazonex nasal spray. Transition from systemic to local SCS can also reveal allergic diseases, such as allergic conjunctivitis and eczema already masked by GCS therapy.

The efficacy and safety of mometasone has not been studied in the treatment of unilateral polyps, polyps associated with cystic fibrosis and polyps that completely cover the nasal cavity.

With the application of Nazonex nasal spray for 12 months there were no signs of atrophy of the nasal mucosa. In addition, mometasone furoate tended to promote normalization of the histological pattern in the study of biopsies of the nasal mucosa.

In the case of unilateral polyps of unusual or irregular shape, especially ulcerated or bleeding, an additional medical examination is necessary.

Drug Interactions

Combination therapy with loratadine was well tolerated by patients. At the same time, there was no evidence of any drug effect on the concentration of loratadine or its main metabolite in blood plasma. In these studies, mometasone furoate in the blood plasma was not detected (with the sensitivity of the method of determining 50 pg / ml).

Reviews

We picked up some reviews of people who used the drug Nazonex:

  • Svetlana. The son at the age of three suddenly suddenly began to appear allergic rhinitis. We immediately appointed NAZONEX, since without hormonal drugs, it was not possible to remove the swelling. Of course, completely safe drug is not, because there are hormones corticosteroids. But with topical application they are almost not absorbed into the blood, and with course irrigation of the nose, we did not experience any side effects. It is very convenient that you should use the Nazonex spray only once a day, before bed. It is a pity that it is so expensive, but we did not find any analogues for ourselves, only this medicine allows the son to breathe easily during the exacerbation of allergies.
  • Alice. To the son , years the doctor has written out nazoneks, on 1shiku in each nostril 1 time a day of 10 days, I had to leave on circumstances, and the treatment of the son was left to be engaged by the grandmother, and instead of once a day pshikala three times a day, was appointed syrup sineupret and antiallergic drops, my grandmother slightly confused and gave drops once a day and spray three times a day, Laura was told how to eat, said that for the future to be attentive, and since already napshikali, no trouble, he is not absorbed into the blood and acts locally, though ENT also calmed down, but I as have represented that instead of a dose the child received a triple dose of a harmonic preparation, also 10 days, Ash hair stand on end!!!! Maybe someone like this was, tell me what's next or how lor and said that there is nothing particularly terrible there?
  • Vladimir. I have been suffering from seasonal rhinitis for the second year. When birches blossom, it is impossible to be in the city. Saves only Nazoneks, who prescribed a doctor. I like that for one application a day I forget about the stuffy nose for a whole day. You can safely walk and not suffer from sneezing, red eyes and swelling of the larynx, which used to be painful for me.
