Fokusin - a drug used to treat disorders of urination, which are provoked by benign prostatic hyperplasia.

Composition and form of release

The preparation is available in the form of hard, gelatin-coated capsules with modified release. Capsules have a size of 1, a transparent body with a dark blue lid and are filled with micropellets, the color of which varies from almost white to white.

Fokusina includes:

  • 0.4 mg of tamsulosin (Tamsulosinum);
  • dispersion of the methacrylate copolymer;
  • microcrystalline cellulose;
  • dibutyl sebacate;
  • polysorbate 80;
  • silicon dioxide colloidal water;
  • talc.

Pharmacological effect

Drug Focussin belongs to the group of alpha-adrenoreceptor blockers.

The active ingredient of Focusin is tamsulosin. It blocks postsynaptic and alpha-1-adrenergic receptors located in the smooth muscles of the prostate gland and the prostatic part of the urethra. This reduces the tone of the neck of the bladder, the muscles of the prostate relax, and the pressure on the urethra decreases, thereby increasing its lumen. Tamsulosin eliminates the symptoms of urinary disorders, reduces obstruction and normalizes bladder function, and also reduces the likelihood of complications of benign prostatic hyperplasia.

Normalizing the work of the bladder, the drug prevents vesicoureteral reflux, causing malfunction in the kidneys. Regular use of Focusus facilitates urination and improves sexual function in those with prostatic adenoma. The drug is adsorbed in the gastrointestinal tract. For better absorption of the drug, it is recommended to swallow capsules after eating.

Indications for use

Focussin is prescribed to treat disorders of the prostate gland that appeared against the background of its hyperplasia.

Contraindications

The instruction to Fokusin prescribes with caution the following groups of people to take medicine:

  • susceptible to chronic renal failure, severe hepatic insufficiency and / or hypertension.

Dosage and route of administration

As indicated in the instructions for use of the capsule, the focussin should be taken after the first meal, without chewing, with enough water.

Adverse Reactions

Like most other drugs, Foccusin in some cases can cause unwanted adverse reactions of varying severity, which include:

  • gastrointestinal disorders, which are expressed in the form of disorders of the stool, attacks of nausea and vomiting (these symptoms occur infrequently);

  • disorders of the nervous system, expressed as dizziness (often), headaches (infrequently), loss of consciousness (extremely rare);
  • violations of the sexual system, which can sometimes be expressed in the form of retrograde ejaculation (i.e., ejaculation characterized by ejection semen in the opposite direction) and very rarely in the form of a long painful erection, not associated with sexual arousal (priapism);
  • violations of the respiratory system, organs located in the chest and anatomical space in the middle of the thoracic cavity (the most common manifestation of this group of side effects is rhinitis).

Other side effects, which are noted against the background of taking Fokusina, are infrequent asthenia, and also the narrowed pupil syndrome (unstable iris). The latter can develop during the surgical treatment of cataracts in patients who have been taking tamsulosin for a long time.

Overdose

To date, there has been no recorded case of an overdose of Focus. But theoretically, when taking too high a dose of the drug, there is a possibility of irregularities in the work of cardiac activity.

In such a case, it is recommended that the patient enter a vasoconstrictor and perform cardiotropic therapy. Also, gastric lavage and the use of enterosorbents can be effective. Hemodialysis in this case is not effective.

  1. At the first signs of orthostatic hypotension (dizziness, weakness), the patient should be seated or stowed.
  2. Before starting therapy with Focussin, other diseases that can cause the same symptoms as benign prostatic hyperplasia should be excluded. Before the start of treatment and regularly during therapy, a prostate examination (digital rectal examination, PSA determination) should be performed.
  3. As with the use of other alpha1-adrenoblockers, when treated with Focussin, in some cases there may be a decrease in blood pressure, which can lead to a fainting condition. The drug Fokusin should be used with caution in patients with a predisposition to orthostatic hypotension.
  4. There are reports of cases of development of prolonged erection and priapism against the background of therapy with alpha 1-adrenoblockers. If the erection persists for more than 4 hours, you should immediately seek medical help. If priapism therapy has not been carried out immediately, it can lead to damage to the tissues of the penis and an irreversible loss of potency.
  5. With the development of angioedema, as well as other immunological reactions, such as Stevens-Johnson syndrome, the drug should be discontinued immediately. The patient should be under the supervision of a doctor before the elimination of this pathological condition; re-administration of tamsulosin is not allowed.
  6. When performing surgical operations for cataracts in some patients who received tasmulosin hydrochloride at the time of surgery and in the past, there was a development of the syndrome of intraoperative instability of the iris (narrow pupil syndrome). The emergence of the syndrome of intraoperative instability of the iris can lead to an increased risk of complications from the visual organ during and after the operation.
  7. Tamsulosin hydrochloride should not be used in combination with potent inhibitors of the CYP3A4 isoenzyme (eg, ketoconazole, voriconazole) in patients with the phenotype of "slow metabolism" of the isoenzyme CYP2D6.

Care should be taken when driving vehicles and occupations requiring increased concentration of attention and speed of psychomotor reactions. Compatibility with other drugs

When using the drug, it is necessary to take into account the interaction with other drugs:

  1. The simultaneous use of tamsulosin with other alpha1-blockers can lead to a decrease in blood pressure.
  2. Diclofenac and indirect anticoagulants (warfarin) slightly increase the rate of excretion of tamsulosin.
  3. Cimetidine increases the concentration of tamsulosin in plasma, and furosemide - reduces (no significant clinical significance).
  4. With simultaneous use of tamsulosin with atenolol, enalapril or theophylline, there were no signs of drug interaction.
  5. Caution should be exercised while simultaneously using tamsulosin with potent and moderately active inhibitors of the CYP3A4 isoenzyme.
  6. In vitro, the administration of diazepam, propranolol, trichloromethiazide, chloromadinone, amitriptyline, diclofenac, glibenclamide, simvastatin and warfarin did not change the free fraction of tamsulosin in the blood plasma. Tamsulosin also had no effect on free fractions of diazepam, propranolol, trichloromethiazide and chloromadinone.
  7. The simultaneous use of tamsulosin and potent inhibitors of CYP3A4 can cause an increase in the systemic exposure of tamsulosin. Simultaneous use of tamsulosin with ketoconazole (a potent inhibitor of the isoenzyme CYP3A4) led to an increase in the indices of AUC and Cmax of tamsulosin by 2.8 times and 2.2 times, respectively.
  8. Tamsulosin should not be used in combination with potent inhibitors of the CYP3A4 isoenzyme in patients with a slow metabolism phenotype on the CYP2D6 isoenzyme.
