Ultop refers to antiulcer drugs designed to regulate secretion, the activity of juice secreted by the stomach.

The active ingredient, omeprazole, is a substance that actively forms bonds with enzymes on the surfaces of gastric secretion cells, which leads to a decrease in their activity. Ultop is used in the complex therapy of diseases of the intestinal tract, ulcerous manifestations. An important feature of the drug is that for the effectiveness of its action, it is necessary to create a concentration of the main component, which is achieved with proper administration of the drug.

Ultop is available in two dosage forms, principally different by their introduction into the body: enteric-soluble capsules and lyophilizate for the preparation of a solution for infusions.

Composition and form of release

Ultop is available in the form of pink-white capsules in blisters for 7, 14 or 28 pieces. Blisters are packed in a cardboard box, detailed instructions with a description are attached to the preparation. When the capsule is opened, white pellets are found inside with a slight yellowish or pinkish tinge.

  • Each capsule contains 10, 20 or 0 mg of active active ingredient - Omeprazole.

Auxiliary components are sucrose, corn starch, titanium dioxide, sodium lauryl sulfate, magnesium hydroxycarbonate, dyes.

Pharmacological effect

Irreversibly binds to the enzyme H + / K + -ATPase, known as a proton pump. Oppresses its activity, which leads to a decrease in basal and stimulated secretion. The effect of the drug depends on the dose. The effect is manifested after 1 hour, and maximum - after 2 hours and lasts 24 hours. At the end of the treatment, the secretion is normalized within a week. At reception of 20 mg acidity decreases on 90% and on 94% at reception of 40 mg.

Suction occurs in the small intestine for 3-6 hours. Cmax in the blood after, -1 h. Biological availability is 35%. Bioavailability increases in the elderly and in case of impaired liver function. Metabolised in the liver. It is excreted by the kidneys (70%) and with bile (30%).

Indications for use

Capsules, enteric, soluble 10 mg:

  • short-term treatment of diarrheal disorders of non-ulcerate nature;
  • long-term maintenance therapy for the prevention of relapses of gastroesophageal reflux disease;
  • long-term maintenance therapy in order to prevent relapses of peptic ulcer of the stomach and duodenum.

Capsules enteric-soluble 20 mg and 40 mg, lyophilizate:

  • gastroesophageal reflux disease (GERD), including non-erosive forms of reflux disease and reflux esophagitis;
  • erosive and ulcerative lesions of the stomach and duodenum caused by the use of NSAIDs, ulcers on the soil of stress;
  • Zollinger-Ellison syndrome and other pathological conditions, the cause of which is increased gastric secretion;
  • peptic ulcer of the stomach and duodenum (in acute stage and as a component anti-relapse therapy), including ulcers associated with Helicobacter pylori (as an element complex therapy).

Lyophilizate is also used to prevent the penetration of stomach contents with high acidity into the respiratory tract during general anesthesia (Mendelssohn syndrome).

Contraindications

The drug Ultop has a number of limitations and contraindications, so therapy can be started only after consulting a doctor and carefully studying the attached annotation. Tablets are contraindicated in the following cases:

  1. Hereditary intolerance to fructose;
  2. Hypersensitivity to Ultopa or its components;
  3. Any term of pregnancy and the period of breastfeeding;
  4. Patients of childhood;

  6. Individual intolerance to Ultop or its components;
  7. Deficiency of sugar or isomaltase.

Intended use for pregnancy and lactation

The safety of omeprazole for a pregnant woman and fetus is not established, therefore Ultop capsules are not prescribed for future mothers. If peptic ulcer is worsening, a pregnant woman should consult her doctor to choose an effective and safe preparation.

When taking the drug, women of reproductive age should be carefully protected from possible pregnancy, and if it does occur, the capsules are immediately discontinued and referred to the doctor.

In the period of breastfeeding, the use of Ultop capsules is not recommended, since omeprazole may be excreted in breast milk and enter the child's body. If necessary, treatment with the drug lactation is completed and the child is transferred to artificial nutrition.

Dosage and route of administration

As indicated in the instructions for use Ultop taken inside, before eating (because eating slows the absorption of the drug), washed down with a small amount of water (the contents of the capsule can not be chewed).

  1. With gastric ulcer in the acute phase and erosive-ulcerative esophagitis - 20-40 mg / day for 4-8 weeks.
  2. With duodenal ulcer in the phase of exacerbation, Ultop is prescribed at 20 mg / day for 2-4 weeks. In resistant cases it is possible to increase the dose to 40 mg / day.
  3. With Zollinger-Ellison syndrome and other pathological conditions associated with increased gastric secretion the dose is selected individually depending on the initial level of gastric secretion, usually starting at 60 mg / day. If necessary, the dose is increased to 80-120 mg / day, in this case it is divided into 2-3 doses.
  4. With gastroesophageal reflux disease, patients with moderate inflammation of 1 caps. 20 mg 1 time / day in the morning, before breakfast for 4-8 weeks. To ensure the following dosing regimen, it is possible to take the drug at another registered dosage (Ultop, enteric-soluble capsules 40 mg). The duration of the main course of treatment is usually 4-8 weeks. After healing of erosive esophagitis, supportive treatment for 26-52 weeks is shown, with severe esophagitis - for life. In cases of relapse of severe GERD or resistant GERD, the treatment can be extended up to 4-8 weeks.
  5. With non-ulcer dyspepsia, the usual dose is 10 mg to 20 mg 1 time / day for 2-4 weeks. If after 4 weeks of taking the drug the condition does not improve, or the symptoms of dyspepsia appear again soon after stopping the intake, the diagnosis should be revised.
  6. For the prevention of recurrences of peptic ulcer of the stomach and duodenum, 10-20 mg / day are prescribed.
  7. For the prevention of recurrences of gastroesophageal reflux disease - 20 mg / day for a long time. The safety and efficacy of 12-month maintenance therapy have been proven. Application on demand is possible.
  8. To eradicate Helicobacter pylori - 20 mg 2 times / day for 7 or 14 days (depending on the treatment regimen used) in combination with antibacterial drugs.

For patients with impaired renal function, dose adjustment is not required.

In patients with impaired liver function, the bioavailability and clearance of omeprazole is increased. In this regard, the therapeutic dose should not exceed 10-20 mg / day.

For elderly patients, correction of the treatment regimen is not required.

Liofilizate for solution for infusion

Infusion solution Ultopa is prepared as follows: lyophilisate is dissolved in 100 ml of 5% infusion solution of glucose (dextrose) or in 100 ml infusion saline. The maximum period of use of the solution on the basis of dextrose is 6 hours, the solution based on physiological solution is 12 hours.

If for patients with reflux-esophagitis or peptic ulcer of the stomach or duodenum for any reasons, oral Ultopa is not possible, it is administered intravenously at a dose of 40 mg 1 time per day. In patients with Zollinger-Ellison syndrome, it is recommended that the drug is initially administered intravenously in a dose of 60 mg. Selection of the dose is done individually, sometimes even higher dose is needed. If the daily dose is more than 60 mg, it is administered in 2 divided doses.

The infusion solution of the drug is administered intravenously drip for 20-30 minutes. It is desirable to use the solution immediately after its preparation.

In elderly patients or with renal dysfunction, when capsules or lyophilizate Ultop are used, there is no need for dose adjustment. In patients with liver dysfunction, the clearance of omeprazole and its availability is increased, so the daily dose should not exceed 10-20 mg.

special instructions

Before you start using the drug, read the special instructions:

  1. The intake of food slows the absorption of omeprazole, so the drug is taken before meals.
  2. Caution is necessary to take Ultop in patients with cirrhosis; the daily dose should not exceed 20 mg.
  3. Before the start of treatment, it is necessary to exclude the presence of malignant process (especially with gastric ulcer), since treatment, masking the symptoms, can delay the setting of the correct diagnosis.
  4. In special cases, if there are difficulties with swallowing the whole capsule, you can swallow its contents after opening or resorption capsules, and it is also possible to mix the contents of the capsule with a slightly acidified liquid (juice, yogurt) and use the resulting suspension for 30 min.
  5. Also with caution, it is necessary to take the Ultop drug in patients with impaired renal function. In patients on dialysis, the pharmacokinetic parameters of omeprazole do not change.
  6. Ultop contains sucrose, so the drug is not recommended for patients with glucose malabsorption syndrome and / or galactose or sucrose / isomaltose deficiency.

Interaction with other drugs

Omeprazole is able to reduce the absorption of ampicillin esters, salts of drugs, since omeprazole normalizes the acid-base balance of the stomach.

The drug may reduce the excretion of substances that affect blood coagulability, phenytoin, or, conversely, increase the concentration of these substances. With this effect, the dose of the drug decreases. Clarithromycin accelerates the absorption of omeprazole.

Reviews

We suggest you read the reviews of people who used Ultop:

  1. Natalia. I drank the course "Ultopa" during an exacerbation of gastritis, but it did not notice its difference from inexpensive analogues in action. Rather, even on the contrary, it was necessary to drink it twice in order to make it easier and longer not to suffer heartburn and stomach pain. For the future, when needed, with a quiet conscience I will buy more budgetary preparations of omeprazole.
  2. Ksyusha. I suffer from gastritis for a long time. Especially never treated, only pain removed, when there was no strength to endure. But the time has come when the stomach began to hurt almost constantly. Then I went to the hospital. There I was advised "Ultop". Drinking the drug for two weeks, I felt that I felt much better, the pain was almost gone. One of the few drugs that not only eliminates pain, but also cures. One thing I do not like is capsules, I can not stand them.
  3. Stanislav. Ultopa was prescribed to me in the hospital after a perforated ulcer (they had an operation) since then my stomach does not bother me, though I drink the ultop all the time now, but it's better than the horror that I was.
