Theopack is a drug with bronchodilating effect, reducing the contractile activity of smooth muscles, increasing renal blood flow and lowering pressure in a small circle of blood circulation.

The systematic use of this medication may inhibit the adhesion of platelets, dilate the cardiac vessels and have a diuretic effect. Frequent use of Teopek in higher dosages causes stimulation of the nervous system, which is very similar to the effect of caffeine.

Composition and form of release

The drug form of the release of Teopek is prolonged-action tablets: white, biconvex, let's say a weak specific odor; 300 mg - with a risk (10 pcs. in contour mesh packaging, in a cardboard bundle of 5 packs; for 40 pcs. in plastic cans, in a cardboard bundle of 1 can).

Composition of 1 tablet:

  • active substance: anhydrous theophylline - 100, 200 or 300 mg;
  • auxiliary components (100/200/300 mg): composite polymeric carrier (interpolymer complex of macrogol and polymethacrylic acid) - 39/78/96 mg; calcium stearate - 1/2 / 4 mg.

pharmachologic effect

Theopack affects various functions of the body. It has a stimulating effect on the central nervous system, although less pronounced than caffeine; increases the contractile activity of the myocardium (cardiac muscle). Multiple extends peripheral, coronary (cardiac) and renal vessels, has a moderate diuretic (diuretic) effect, inhibits aggregation (prevents gluing) of platelets, inhibits the release of mast cells from allergic mediators.

Particularly important is the ability of the theopek to exert a bronchodilator effect. In the mechanism of action of theopec, a certain role is played by inhibition of phosphodiesterase and accumulation in the tissues of cyclic 3-5-adenosine monophosphate. Accumulation in cAMP cells inhibits the myosin connection with actin, which reduces the contractile activity of smooth muscles (muscles vessels and internal organs) and promotes, in particular, the relaxation of the bronchi and the removal of bronchospasm (narrowing of the lumen of the bronchi). The ability of theopek to inhibit the transport of calcium ions through the "slow" channels of the cell membranes also results in relaxation of the musculature. Of particular importance in the molecular mechanism of the action of the theopec is its recently discovered ability to block adenosine (purine) receptors.

The combination of teopek with a composite polymer carrier provides a dosed release of theophylline in the gastrointestinal tract. Peak concentrations of theophylline in the blood plasma observed after 3-6 hours after taking the tablet inside, and the total duration of detection of the drug in plasma reaches 12-24 hours.

Indications for use

Contraindications

Absolute:

  • epilepsy;
  • pregnancy;

Intended use for pregnancy and lactation

Experiments conducted on animals have shown that theophylline has very high activity - it passes through the placenta and penetrates into breast milk.

The experiments were not carried out on pregnant women, and it is still unclear the effect of theophylline on the woman and the fetus. For this reason, physicians avoid appointing Theopek during the gestation of the child and GW. If the doctor, after analyzing the relationship between the benefits and harm of the drug, considers the use of Teopek justified, then he can be prescribed for treatment. Dosage and route of administration

  • The daily dose of the drug is divided into two doses, the interval between which is 12 hours.
  • The optimal dose is selected individually for each patient. When choosing a dose, the nature of the disease, weight and age of the patient are taken into account.
  • The average daily dose for children 3-7 years is 100 mg, for children 7-12 years - 200 mg, for adults and teenagers over 14 years - 300 mg.
  • The initial dose of the drug Teopek is half the standard daily dose in two divided doses. If the first day of treatment, there are no side effects, the patient is transferred to a full daily dose.

The duration of the course of treatment varies from 2 weeks to 2 months and depends on the therapeutic effect obtained and the features of the course of the specific disease.

Side effects

Against the background of taking Tablet Teopec, it is possible to develop negative pathological reactions from various organs and systems, which include:

  • Allergic reactions - skin rash and itching, the development of an allergic fever with an increase in body temperature.
  • Nervous system - periodic dizziness, headache, anxiety, irritability, agitation, trembling (tremor) of the hands.
  • The urinary system is the appearance of protein (albuminuria), blood (hematuria) in the urine, an increase in its volume (increased diuresis).
  • Cardiovascular system - the appearance of a palpitation, the rapidity of contractions of the heart (tachycardia), rhythm disturbance (arrhythmia), decrease the level of systemic arterial pressure (hypotension), pain in the heart, the increase in angina attacks, sensations of hot flashes to the face.
  • The digestive system - nausea, accompanied by periodic vomiting, throwing the contents of the stomach into the esophagus (reflux), pain in the stomach, heartburn, worsening appetite, exacerbation of peptic ulcer.

Usually the development of negative reactions is reversible, and is also a dose-dependent state. When such reactions occur, the need for discontinuing the drug is determined by the attending physician.

Overdose

Symptoms: decreased appetite, gastralgia, diarrhea, nausea, vomiting (including with blood), gastrointestinal bleeding, tachypnea, hyperemia facial skin, tachycardia, ventricular arrhythmias, insomnia, motor excitement, anxiety, photophobia, tremor, convulsions. In severe poisoning, epileptic seizures, hypoxia, metabolic acidosis, hyperglycaemia, hypokalemia, decrease in arterial pressure, confusion, renal failure with myoglobinuria.

Treatment: drug cancellation, gastric lavage, the appointment of activated carbon, laxative drugs, washing the intestine with a combination of polyethylene glycol and electrolytes, forced diuresis, hemosorption, plasmosorption, hemodialysis (efficacy is not high, peritoneal dialysis is not effective), symptomatic therapy (including metoclopramide and ondansetron in vomiting). If seizures occur, maintain airway patency and conduct oxygen therapy. For relief of seizure - iv diazepam, 0.1-0.3 mg / kg (but not more than 10 mg). With severe nausea and vomiting - metoclopramide or ondansetron (IV).

In pediatric practice and elderly patients, Teopek can be appointed for life indications.

Rectal forms are used with caution in diagnosed diseases of the rectal area of ​​the intestine (and with suspicion of them), with diarrheal syndrome. Smoking patients have a decrease in the effectiveness of the medication.

With severe coronary insufficiency (angina pectoris, myocardial infarction in acute period), ventricular extrasystole, total vascular atherosclerosis, obstructive hypertrophic cardiomyopathy, peptic ulcer disease, chronic heart failure, severe hepatic pathology, increased convulsive activity, hypertrophy prostate, gastroesophageal reflux, pregnancy, thyrotoxicosis, uncontrolled hypothyroidism (cumulation is recorded), the drug is used with caution.

Simultaneous therapy with other drugs, the active substance in which the xanthine derivatives act, is inadmissible. In elderly patients with severe concomitant pathology, with viral infectious lesions, with diseases of the hepatic and cardiovascular systems, correction of the treatment regimen is required.

Interaction with other drugs

  1. Theopek potentiates the action of diuretics and suppresses the therapeutic effects of beta-blockers and lithium carbonate.
  2. Enterosorbents and antidiarrheal agents reduce the absorption of theophylline.
  3. The drug is administered with caution together with anticoagulants.
  4. Teopack is not recommended to be used simultaneously with other xanthine derivatives.
  5. Diphenin, isoniazid, sulfinpyrazone, phenobarbital, phenytoin, carbamazepine and rifampicin decrease the effectiveness of the drug (an increase in the dose of Teopek may be required).
  6. With simultaneous application with mineralocorticosteroids, glucocorticosteroids and drugs that excite the central nervous system, the risk of side effects of these drugs increases.
  7. Allopurinol, isoprenaline, oral contraceptives, influenza vaccines, macrolide antibiotics, cimetidine, enoxacin, Linkomycin and beta-blockers can increase the intensity of the drug (you may need to reduce the dose of Teopek).
