Orsotene for weight loss

Orsotene is a medicament that reduces the absorption of fats in the intestine. It has in its composition an active substance called orlistat.

This component inhibits gastric and pancreatic lipase. Due to the occurrence of covalent bonds, the main ingredient prevents the cleavage of lipids found in food.

The therapeutic effect of Orsotene is carried out without absorption into the systemic circulation. The effect of orlistat leads to an increase in the fat content in the feces after 24-48 hours after taking Orsoten. After the drug is withdrawn, the fat content in the stools usually returns to the initial level after 48-72 hours.

Clinical and pharmacological group

The drug for the treatment of obesity is an inhibitor of gastrointestinal lipases.

Pharmacological effect

The energy value of fats is 9.3 kcal / g, which is almost twice as high as that of carbohydrates and proteins. Entering the gastrointestinal tract in the form of triglycerides, fats of vegetable or animal origin are cleaved by the enzyme with lipase to free fatty acids. They are used by the body as a source of energy, superfluous stored in reserve, like fat accumulation.

The specific blocker lipase orlistat is able to form strong bonds with these enzymes, thereby depriving them of the ability to cleave triglycerides. At the same time fats pass through the digestive system, not digesting, and are excreted with feces and partially with urine. The principle of the action of orlistat is the same as that of any lean diet - consumption by the body for energy needs of fat stores accumulated earlier.

The results of clinical studies of orsotene showed that its use for 3 months in combination with a low-calorie diet provides a decrease in body weight by 10 - 15%, while the use of dietary methods alone - only 5 - 7%.

Pharmacokinetics

Orlistat has a therapeutic effect without absorption into the systemic bloodstream. After 8 hours after internal application, the concentration of the drug in the blood plasma is less than 5 ng / ml. At the same time, its minimal absorption is confirmed by the absence of signs of cumulation.

The drug is 99% bound to albumin and lipoproteins, in minimal quantities found in red blood cells. It is metabolized mainly in the intestine. Forms pharmacologically inactive metabolites M1 and M3. About 97% of orlistat is excreted along with feces, 83% in unchanged form. Within 24-48 hours after the beginning of taking the drug in the feces, the fat content increases. The time for complete elimination of the drug is 3-5 days.

Indications for use

Long-term treatment of obese patients with a body mass index (BMI) ≥30 kg / m2, or overweight patients (BMI ≥28 kg / m2), incl. having associated with obesity risk factors, in combination with a moderately low-calorie diet.

If food is missed or if the food does not contain fat, you can skip the intake of orlistat.

  • Doses of orlistat more than 120 mg 3 times / day do not enhance its therapeutic effect. Duration of therapy - no more than 2 years.

Correction of the dose is not required for elderly patients or patients with impaired liver or kidney function.

The safety and efficacy of orlistat treatment for children and adolescents under the age of 18 years is not established.

Side effects

Most often during the reception of Orsoten develop violations of the digestive tract, associated with an increased amount of fat in the stool. In most cases, these disorders are mild and transient and develop during the first 3 months of therapy. With prolonged therapy, the number of cases of side effects decreases.

During the application of Orsotene, the following disorders may develop:

  1. Metabolism: hypoglycemia (with type 2 diabetes mellitus);
  2. Central nervous system: a sense of anxiety, headache;
  3. Skin: very rarely - bullous rash;
  4. Allergic reactions: rarely - angioedema, itching, urticaria, rash, anaphylaxis, bronchospasm;
  5. Digestive system: urge to defecate, flatulence with discharge from the rectum, oily / fatty stools, oily discharge from the rectum, a liquid and / or soft stool, steatorrhea (inclusion of fat in the stool), a feeling of discomfort and / or pain in the abdomen and in the rectum, stool incontinence, increased defecation, imperative urges to defecate, defeat gums and teeth; very rarely - cholelithiasis, diverticulitis, hepatitis (possibly severe), increased alkaline phosphatase and liver transaminase activity;
  6. Other: a feeling of fatigue, dysmenorrhea, flu-like syndrome, upper respiratory tract infections and urinary organs.

Overdose

At the moment, cases of an overdose of Orsotene are not described. Significant side effects from the ingestion of the active substance in a dose of 800 mg, several doses up to 400 mg daily, for 15 days are not found.

There was no evidence of an increase in the side effect of the drug when patients with a diagnosis of obesity received a three-fold dose per day of 240 mg of orlistat over a six-month period.

In case of an overdose of the drug, the patient should be observed throughout the day.

special instructions

To ensure adequate nutrition of patients it is recommended to take multivitamin preparations.

If after 12 weeks of therapy there was no decrease in body weight, at least 5%, orlistat should be stopped.

Decreased body weight during treatment with orlistat may be accompanied by an improvement in carbohydrate metabolism compensation in patients with type 2 diabetes mellitus, which can reduce the dose of hypoglycemic drugs.

Patients should adhere to dietary guidelines. They should receive a balanced, moderately low-calorie diet, containing no more than 30% of calories in the form of fats. Daily intake of fat should be divided into three main meals.

Orlistat is effective for prolonged monitoring of body weight (weight loss, maintenance at an appropriate level and prevention of repeated weight gain). Treatment with orlistat leads to an improvement in the profile of risk factors and diseases associated with obesity (including hypercholesterolemia, violation glucose tolerance, hyperinsulinemia, arterial hypertension, type 2 diabetes mellitus), and a reduction in the amount of visceral fat.

The likelihood of developing side effects from the gastrointestinal tract may increase if orlistat is taken amid a diet rich in fats (for example, 2000 kcal / day,> 30% of the daily calorie intake comes in the form of fats, which is approximately 67 g of fat). Patients should be aware that the more precisely they observe a diet (especially regarding the allowed amount of fat), the less likely the development of adverse reactions. A low-fat diet reduces the likelihood of adverse reactions from the gastrointestinal tract and helps patients control and regulate fat intake.

Drug Interactions

When using the drug, it is necessary to take into account the interaction with other drugs:

  1. Increases the bioavailability and hypolipidemic effect of pravastatin, increasing its concentration in plasma by 30%.
  2. Interaction with amitriptyline, biguanides, digoxin, fibrates, fluoxetine, losartan, phenytoin, oral contraceptives, phentermine, nifedipine (including delayed-release), sibutramine, furosemide, captopril, atenolol, glibenclamide, or ethanol was not observed.
  3. Patients receiving warfarin or other anticoagulants and orlistat may experience a decrease in the level of prothrombin, an increase in INR, leading to changes in haemostatic parameters.
  4. Treatment with orlistat may potentially interfere with the absorption of fat-soluble vitamins (A, D, E, K). If multivitamins are recommended, they should be taken no earlier than 2 hours after taking orlistat or before bedtime.
  5. A decrease in body weight can improve metabolism in diabetic patients, and as a result, it is necessary to reduce the dose of oral hypoglycemic agents.
  6. Patients receiving amiodarone should be more carefully monitored and monitored by the ECG; cases of a decrease in the level of amiodarone concentration in the blood plasma are described.
  7. With the simultaneous administration of orlistat and cyclosporin, there was a decrease in the level of cyclosporin concentration in the plasma blood, therefore it is recommended to conduct a more frequent determination of the level of cyclosporine concentration in the blood plasma.

Reviews

We picked up some reviews of slimming people using the drug Orsoten:

  1. Novel. Because of health, I had to start taking Orsoten. For the first month I got rid of 4 kg, then the weight loss stopped. I added fitness, and for the next 3 months I dropped another 6 kg.
  2. Yana. I decided on this drug, sitting on the Internet for three days and reading about this type of diet pills. Many reviews, of course more negative, but I want to try, because without the help of the pill I feel I can not do. Like and I want to eat less, but quickly break, there is no incentive or something, and so buy expensive capsules, already have with them and eat less and observe more proper nutrition. My weight is not critical, but over the past two years, a lot of extra kilograms I have collected, I have to do something. I'll start with taking Orsotene, tomorrow morning in the pharmacy)
  3. Alexandra. Tried to take the drug Orsotene and a more expensive analogue - Xenical. According to my observations, Orsoten is more effective, despite the doctors' comments. With refusal of all harmful, sweet, for a year on a diet with this medicine has thrown off 12 kg without physical exercises.
  4. Masha. After taking notice of noticeable changes in the volume and clothes, and outwardly it became apparent, while I drank half, took 42 tablets. I think the results will please me. In parallel, I try to do cardio every day, if possible, and limit myself to sweet. At this stage, I want to say that the drug really works. All good double digits on the scales!

Reviews of doctors about Orsotene for weight loss

In general, the opinions of the treating specialists are positive with regard to the described remedy. Endocrinologists and nutritionists confirm the safety of orlistat and its main advantage is low absorption to the blood. In addition, many doctors prefer to use Orsoten in the practice of treatment of type 2 diabetes, emphasizing its unique ability to reduce the concentration of cholesterol, improve metabolism and accelerate metabolism.

Nevertheless, there is an important nuance: treatment with this drug must necessarily be combined with a low-calorie diet and minimal physical exertion (at least 20 minutes per day in the form of charging).
Otherwise, the intake of fat in the body will quickly recover after the cancellation of Orsotene and the lost excess weight will return. Moreover, it is desirable already during the therapy with orlistat to exclude from the diet simple carbohydrates and rich in lipid connections to food, so that the digestive system can more easily cope with the load in the future.

Analogues

To combat excess kilograms of cheap Orsoten analogues are the following:

  1. Xenical. A drug from one pharmacological group with Orsotene, also contains orlistat.

  3. Orsotene Slim. Dosage Orsoten (Orsoten) with a lower content of active ingredient in one capsule (60 mg).
  4. Alli. Lipase inhibitor. The mechanism of action is caused by a violation of the breakdown of fats from food and a reduction in their absorption from the gastrointestinal tract.

Before using analogues, consult your doctor.
