The principle of the operation of the Tri -grim tablets is explained by the combination of torasemide with the Na + / 2Cl- / K + transporter, which is located in the apical membrane of the thick segment of the ascending loop of Henle. That is why it can be said that in the human body it decreases substantially or almost completely slows down, and also reabsorption of sodium ions is prevented.

The result of this process is a decrease in the osmotic pressure of the intracellular fluid, as well as the reabsorption of water. In responses to Trigrim, it was noted that the drug, due to its antialdosterone action, which is much less pronounced than in furosemide, practically does not provoke hypokalemia, despite the high activity of its components and prolonged action.

Indications for use

Receiving of tablets Trigrim is indicated for the complex treatment of arterial hypertension (high blood pressure), the purpose of which is to reduce the volume of circulating blood.

Also, the drug is used to reduce the severity of edema of soft tissues of various locations and origin (edema caused by heart failure, liver cirrhosis, kidney or lung pathology).

Contraindications

The intake of tablets is contraindicated with several pathological and physiological states of the organism, which include:

  • age under 18 years (for this group of patients, efficacy / safety was not established);
  • pregnancy and lactation period (there is no information on the efficacy / safety of Trigrim during breastfeeding);
  • individual intolerance of the drug components, as well as hypersensitivity to sulfonamides;
  • arterial hypotension;

Dosage and route of administration

  1. With primary arterial hypertension, the dose is 2.5 mg 1 time / day. If necessary, the dose can be gradually increased to 5 mg / day. According to studies, a dose of more than 5 mg / day does not lead to a further decrease in blood pressure. The maximum effect is achieved after approximately 12 weeks of continuous treatment.
  2. With congestive heart failure prescribe a dose of 5-20 mg 1 time / day. If necessary, the daily dose can be gradually increased, doubling it, a maximum of 200 mg.
  3. With swelling, the drug is prescribed in a dose of 5 mg 1 time / day. If necessary, the dose can be gradually increased to 20 mg 1 time / day. In some cases it is recommended to apply up to 40 mg / day.
  4. With cirrhosis the drug is prescribed in a dose of 5-10 mg 1 time / day. If necessary, this dose can be gradually increased, doubling it, until an appropriate diuretic action is achieved. Appropriate controlled studies have not been conducted in patients with liver disease with the use of the drug at doses greater than 40 mg / day.
  5. In chronic renal failure, the initial dose is 20 mg / day. If necessary, this dose can be gradually increased, doubling it, until the optimal diuretic effect is achieved. The maximum daily dose is 200 mg.

Older patients do not need a dose adjustment.

Side effects

Trigrim can provoke the following side effects:

  • from the central and peripheral nervous systems: paresthesia in the limbs, convulsions, drowsiness, general weakness, confusion, dizziness and headache;
  • from the cardiovascular system: lowering blood pressure, thromboembolism and circulatory disorders;
  • from the sense organs: noise in the ears, visual disturbances and deafness;
  • from the side of metabolism: violation of electrolyte balance, hypovolemia, increased blood lipids, glucose and uric acid, hypokalemia;

Overdose

When an overdose of the drug shows the following symptoms: increased diuresis, which is accompanied by a violation of the electrolyte balance, hypovolemia, a drop in blood pressure, confusion, drowsiness, collapse, and gastrointestinal disturbances.

For the treatment of overdose, symptomatic therapy is used, the drug is canceled or its dosage is reduced with simultaneous replenishment of lost electrolytes and liquid.

Trigrim enhances the effect of antihypertensive drugs, can enhance the effects of theophylline and curare-like muscle relaxants, and in high doses - toxic action of antibiotics, aminoglycosides and cisplatin, cardiotoxic and neurotoxic effect of lithium, neurotoxic effect of cephalosporins.

With simultaneous or sequential use with angiotensin-converting enzyme inhibitors, a transient fall in the arterial pressure (it is necessary to reduce the dose of torasemide or reduce the initial dose of inhibitors of the angiotensin-converting enzyme).

With the simultaneous use of torasemide with laxatives, glucocorticoids and mineralocorticoids, potassium excretion is possible; with antidiabetic means - the effect of antidiabetics is weakened; with salicylates in high doses - the toxic effect of salicylates increases; with epinephrine and norepinephrine - their vasoconstrictive effect decreases; with colestyramine - there may be a decrease in the absorption of torasemide from the gastrointestinal tract; from probenecid and non-steroidal anti-inflammatory drugs - hypotensive and diuretic action of torasemide may decrease.
