width = Maltofer - contains iron in the form of a polymaltose complex of iron hydroxide.

This macromolecular complex is stable and does not release iron as free ions in the gastrointestinal tract. The structure of Maltofer is similar to the natural iron compound ferritin.

Due to this similarity, iron comes from the intestine into the blood through active transport. The absorbed iron binds to ferritin and is stored in the body, mainly in the liver. Then, in the bone marrow it is included in the hemoglobin.

Pharmacological effect

Maltofer includes iron (III) complex of polymaltose hydroxide, which is stable, does not release free iron ions in the digestive tract.

Its structure is similar to the natural iron compound ferritin, due to which iron (III) enters the blood from the intestine by means of active transport. Iron, which is absorbed from the Maltofer, binds to ferritin, and accumulates in the liver, mostly. After that, it is included in the hemoglobin in the bone marrow.

There is a relationship between the level of absorption of iron and the severity of its deficiency: iron is absorbed the better, the greater its deficiency in the body. The most active process of iron absorption occurs in the thin and duodenum. The part that is not absorbed into the bloodstream is excreted with feces.

The preparation Maltofer foul except for iron contains folic acid, which belongs to group B of vitamins, takes part in the production of nucleic acids, purines, amino acids, pyrimidines, stimulates erythropoiesis (production process erythrocytes).

Indications for use

The instruction for the use of Maltöfer identifies the following indications for use by patients:

  • treatment of iron deficiency without anemia (latent deficiency), symptomatic manifestation of iron deficiency anemia;
  • solution of iron is used when it is not possible to take oral iron preparations, diseases of the gastrointestinal tract;
  • increased need for iron (pregnancy, lactation, blood donation, intensive growth, vegetarianism, elderly age).

Contraindications

The drug is not prescribed for children:

  • with the detected hypersensitivity to any component of "Maltofer";
  • with hemochromatosis or hemosiderosis;
  • with anemia, the cause of which is not iron deficiency (hemolytic, megaloblastic and other);
  • with intolerance to fructose;
  • with the problems of iron utilization - for example, with thalassemia or sidero-achestic anemia;
  • with impaired glucose / galactose uptake;
  • with a lack of isomaltase or sugar in the body.

Use in pregnancy and lactation

The instructions for the use of Maltofer said that to date there are no reports of adverse reactions when taking the drug during pregnancy. According to animal studies, the drug does not pose a threat to the fetus and the mother after the end of the first trimester. There is no information on the effect of the drug in the first trimester of pregnancy.

According to reviews, during the period of breastfeeding the medicine is prescribed only after consulting with the doctor and assessing the potential risk and the estimated benefits to the child and mother. The active component of the composition penetrates into breast milk, its amount has not been studied. Theoretically, the use of the drug by women breastfeeding, will not cause negative reactions for the baby.

Dosage and route of administration

The instructions for use indicate that the Maltoffer is taken orally during or immediately after a meal. The daily dose can be divided into several doses or taken at a time.

Drops can be mixed with fruit, vegetable juices or soft drinks, baby food or baby formula. Light coloring of the mixture does not affect its taste or effectiveness. To accurately measure the dose of drops, the Maltoffer vial / container (tube) should be kept upright. Drops should flow immediately. If this does not happen, you should tap the bottle / container (tube) slightly before the drop appears. Do not shake the vial / container (tube).

Daily dosage for iron deficiency anemia:

  • for premature infants is 1-2 drops, based on one kilogram of body weight, the course of treatment is 3-5 months;
  • for children up to a year - from 10 to 20 drops per day;
  • for children from one year to 12 years - from 20 to 40 drops per day;
  • adults, pregnant women and children over 12 take 40-120 drops a day.

In the presence of latent iron deficiency and for prevention:

  • Children up to a year are prescribed 6 to 10 drops per day;
  • children from one year to 12 years - 10-20 drops;
  • adults and pregnant women from 20 to 40 drops per day.

Syrup

With anemia:

Side effects

When taking chewable tablets, syrup, drops or a solution for ingestion, there are side reactions from the digestive system: possibly staining the stool in Dark color (due to the release of non-sucked iron); very rarely - symptoms of irritation of the digestive tract (nausea, diarrhea, constipation, a feeling of overcrowding, pressure in the epigastric area).

When using Maltoefer in the form of a solution for intramuscular injection, there may be side reactions from some systems and organs:

  • Digestive system: rarely - vomiting, nausea.
  • Local reactions: skin discoloration, tenderness and inflammation at the injection site (if the technique of introducing the solution is disturbed).
  • Organism as a whole: rarely - headache, lymphadenopathy, malaise, joint pain, fever; very rarely - anaphylactic or allergic responses.

Overdose

It is unlikely that there are cases of overdose, glut of iron, or intoxication with the use of the preparation Maltofer because of the low toxicity of the FAT and controlled iron intake.

More likely the development of an overdose with intramuscular injection of Maltofer, which is manifested by symptoms of glut of iron, such as:

  • Sticky and cold sweat;
  • Abdominal pain;
  • Weak pulse;
  • Tachycardia;
  • Nausea;
  • Vomiting;
  • Diarrhea with blood;
  • Pale skin;
  • Convulsions;
  • Oppression of the central nervous system.

To treat an overdose, Maltofer needs to inject deferoxamine intravenously and carry out symptomatic measures aimed at maintaining the functioning of vital organs and systems.

special instructions

Before you start using the drug, read the special instructions:

  1. Drops for ingestion contain sucrose, which can harm the teeth.
  2. When prescribing a patient with diabetes, it should be noted that 1 ml of drops for oral administration contains 0.01 XE.
  3. During treatment with Maltofer, dark stain can be noted, but this has no clinical significance.
  4. Auxiliary substances of sodium methylparahydroxybenzoate and sodium propyl parahydroxybenzoate, in the composition of drops for oral administration, may cause allergic reactions (possibly, delayed type).
  5. Anemia can be caused by infectious diseases or malignant neoplasms. Since iron can only be taken after the root cause of the disease has been eliminated, the relationship between benefit and risk of treatment should be determined.
  6. Dosage form and concentration of preparations Maltofer drops for ingestion are better suited for taking the recommended dose in this age group.

Drug Interactions

The administration of the drug does not affect the results of the detection of hidden blood (with a selective determination of hemoglobin), so there is no need to interrupt treatment.

In studies in rats using tetracycline, aluminum hydroxide, acetylsalicylic acid, sulfasalazine, calcium carbonate, calcium acetate and calcium phosphate in combination with vitamin D3, bromazepam, magnesium aspartate, D-penicillamine, methyldopa, paracetamol and auranofina, no interaction with iron (III) hydroxide polymaltozate.

The interaction of iron (III) hydroxide of polymaltosate with tetracycline and aluminum hydroxide was studied. There was no significant decrease in tetracycline absorption. The concentration of tetracycline in the blood plasma did not drop below the effective level. Absorption of iron from iron (III) hydroxide polymaltozate did not decrease under the influence of aluminum hydroxide or tetracycline. Thus, iron (III) hydroxide polymaltosate can be used simultaneously with tetracycline and other phenolic compounds, as well as with aluminum hydroxide.

Also, no interaction of iron (III) hydroxide of polymaltosate with food components, such as phytic acid, oxalic acid, tannin, sodium alginate, choline and choline salts, vitamin A, vitamin D3 and vitamin E, soybean oil and soybean flour. These results indicate that iron (III) hydroxide polymaltose can be taken during or immediately after ingestion.

It is necessary to avoid simultaneous use of iron preparations for parenteral administration and ingestion, since absorption of iron taken orally slows down.

Reviews

We picked up some reviews of people taking the drug, Maltofer:

  1. Anna. I have a latent iron deficiency, because the digestive system is broken. Because of this, I am treated every year in a hospital. I go to the hospital, they put me a dropper with a solution of Maltofer. Helps quickly, then I feel good, but plus for a year I follow a special diet to prevent GI diseases.
  2. Ivan. A good medicine is maltofer, only for the stomach it is not kosher. Very severe gastritis and ulcers after it, tested on his family. So you can take short courses. Take strictly after eating, as the instructions say. He needs a stomach, too.
  3. Julia. During pregnancy, I was diagnosed with anemia, which could adversely affect the baby. To prevent this, I drank Maltofer Fol with folic acid and was seen by the doctor. We were able to defeat the disease in a couple of months, but the course of admission continues - you need to stabilize the hemoglobin level to give birth to a healthy baby.
