Momato is a topical hormonal drug.

Used to relieve inflammation as a monotherapy for severe allergies. It is also indicated when pathology is used during an infectious course, but when used in combination with a local or systemic antibiotic.

Form of issue and composition

Medicinal form of Momata Reno - dosed nasal spray: a suspension of thick consistency, translucent or opaque, almost white or white (60 and 120 doses in polyethylene bottles with a dispenser, 1 bottle in a cardboard bundle).

Pharmacological effect

The drug has antipruritic, anti-inflammatory, glucocorticoid action. It shows an antiexudative effect. The composition induces the release of proteins that are responsible for inhibiting the phospholipase. That is, the agent acts directly on the structure of the inflammatory mediators, inhibiting the release of their precursor, arachidonic acid.

In the systemic blood stream, a small part falls, if not used in large areas or under an occlusive dressing. Metabolized mometasone in the liver and it also with bile is excreted. The half-life is about 6 hours.

Indications for use

  1. Seasonal and all-the-year-round allergic rhinitis in children from 2 years and adults;
  2. Polyposis of the nose, accompanied by a violation of smell and nasal breathing, in adults;
  3. Acute rhinosinusitis with mild and moderate symptoms in children aged 12 years and adults (provided there are no signs of severe bacterial infection);
  4. Acute sinusitis and exacerbation of chronic sinusitis in children aged 12 years and adults (including the elderly) - as an aid in the conduct of antibiotic therapy;
  5. Prevention of seasonal allergic rhinitis of moderate and severe course in children aged 12 years and adults (2-4 weeks before the expected beginning of the dusting season).

Contraindications

Contraindications are such conditions as:

  • Pink acne;
  • Pregnancy
  • Post-vaccination period, followed by appropriate reactions;
  • Lactation;
  • Hypersensitivity;
  • Tuberculosis or syphilis;
  • Infectious tissue damage;
  • Perioral dermatitis;
  • Age less than 2 years (for ointment).

Use in pregnancy and lactation

Contraindicated. Detailed studies on the use of the drug in pregnant women have not been carried out, but since azelastine hydrochloride may toxic effects on embryos of laboratory animals, the use of the drug in pregnancy and lactation is prohibited.

Dosage and route of administration

The instructions for use indicate that the Momat Rino spray is applied intranasally. Inhalations are carried out with the aid of a dispensing nozzle provided.

Before using the spray for the first time, it is necessary to calibrate it - press the dispensing nozzle 10 times until splashes appear. This indicates the readiness of the drug for use. If the product is not used for 14 days or more, two test strokes in the air before the spray should be made before use.

Side effects

  1. From the side of the nervous system: often - headache, dysgeusia (unpleasant taste) as a result of improper application, namely, with excessive head deflection back during injection; very rarely - dizziness (can be caused by the disease itself).
  2. From the digestive system: rarely - a sensation of irritation of the mucous membrane of the pharynx, nausea.
  3. From the respiratory system: often - nosebleeds, discomfort in the nasal cavity (burning sensation, itching), ulceration of the nasal mucosa, sneezing, pharyngitis, sinusitis, upper respiratory tract infections.
  4. From the immune system: very rarely - hypersensitivity, anaphylactoid reactions, urticaria.
  5. From the skin and subcutaneous tissues: very rarely - a rash, skin itch.
  6. Other: very rarely - fatigue, drowsiness, weakness (can be caused by the disease itself).
  7. With prolonged use of GCS in high doses, it is possible to develop systemic side effects, incl. glaucoma and cataract.

Overdose

Cases of overdose have not been identified to date (with intranasal application). When taking azelastine inside, it is possible to develop violations from the side of the CNS - inhibition, confusion, drowsiness, lowering of pressure, tachycardia.

Treatment is symptomatic. Long-term use of GCS in large doses and in combination with systemic treatment can lead to oppression of the pituitary, hypothalamus, adrenal glands.

As with any long-term treatment, patients who use the Momat Rino Advance nasal spray for several months and longer should periodically undergo an examination the doctor for possible changes in the nasal mucosa, perforation of the nasal septum (very rarely) and the possible development of systemic side effects. In case of development of a local fungal infection of the nose or throat, it may be necessary to stop the therapy with the Momat Rino Advance nasal spray and carry out special treatment. In the case of the development of persistent irritation of the nasopharynx, it is necessary to solve the problem of discontinuing therapy.

The abolition of SCS systemic action in such patients can lead to an insufficient adrenal function, the subsequent restoration of which can take up to several months. If signs of adrenal insufficiency appear, the systemic GCS should be resumed and other necessary measures taken.

Patients undergoing GCS treatment have potentially decreased immune reactivity and should be warned about the increased risk of infection in case of contact with patients with some infectious diseases (for example, chicken pox, measles), as well as the need for medical advice if such a contact has occurred.

When using intranasal steroids in doses exceeding recommended, or in recommended doses in sensitive patients, systemic effects of GCS and suppression of adrenal function may develop. If such changes occur, the use of the Momat Rino Advance nasal spray should be gradually stopped, in accordance with the procedures adopted to stop oral glucocorticosteroids.

GCS for nasal and inhalation use can cause the development of glaucoma and / or cataracts. Therefore, close monitoring of patients with vision changes, as well as for patients who have previously been noted increased intraocular pressure, glaucoma and / or cataracts.

When there are signs of severe bacterial infection (eg, fever, persistent and sharp pain on one side of the face or toothache, swelling in the orbital or periorbital area) immediate medical advice is required.

Drug Interactions

Mometasone furoate:

  • Combination therapy with loratadine was well tolerated by patients. At the same time, there was no evidence of any drug effect on the concentration of loratadine or its main metabolite in blood plasma. In these studies, mometasone furoate in the blood plasma was not detected (with the sensitivity of the method of determining 50 pg / ml).

Azelastine:
