Form of issue and composition

The medicinal form of Irifrin is a drop. They are a clear solution of a colorless or yellowish hue. Are issued in special 5 ml plastic bottles with a dispenser, which, in turn, are packed in cardboard boxes.

  • Drops are of different percentage ratios of 2.5% and 10%. The active ingredient is phenylephrine hydrochloride.
  • In 1 ml of a 2.5% solution contains 25 mg of the active ingredient, and in 1 ml of a 10% solution of 100 mg.

Additional components in the drug: benzalkonium chloride, edetate disodium, hypromellose, sodium metabisulphite, citric acid, sodium citrate dihydrate, distilled water.

Pharmacological effect

Active substance Irifrin phenylephrine is an alpha-adrenomimetic, accordingly, it affects the smooth muscles of blood vessels. When using phenylephrine in the form of eye drops, the drug acts only on the vessels of this organ. If phenylephrine is administered intravenously or subcutaneously, it affects all the vessels in the human body, as well as the heart.

The application of drops of Irifrin to the mucous membrane of the eye causes a dilatation of the pupil, increases the outflow of the intraocular fluid, and also narrows the blood vessels of the conjunctiva. The constriction of the conjunctival vessels ensures the disappearance of the reddening of the eye, as a result of which the drug is used in the therapy of the "red eye" syndrome. The increase in the outflow of intraocular fluid provides an improvement in the state of the eye in glaucoma. And the dilated pupil caused by drops of Irifrin is used for preoperative preparation or during surgery on the eyes.

The narrowing of the vessels of the eye occurs within 30 to 90 seconds after the drops are applied to the conjunctiva. The dilatation of the pupil occurs 10 to 60 minutes after a single instillation of the solution, and remains for 2 hours with 2.5% irifrin or 3-6 hours with 10% drops.

Indications for use

Both varieties of Irifrin are intended for use in ophthalmology. The drugs are administered under the following conditions:

  • conjunctivitis of different etiology;
  • in the treatment of glaucoma and cyclical crises, accompanied by irregularities in intraocular pressure;
  • bursting sosudiki in the eyes ("red eye" syndrome);
  • iridocyclitis (extensive inflammatory process in the ciliary body of the eye);
  • the need to expand the pupil in the process of conducting diagnostic procedures;
  • with the aim of eliminating the accommodation spasm, that is, reducing the ciliary muscle fibers;
  • implementation of a provocative test in case of suspicion of a closed-angle glaucoma;
  • the need to expand the pupil in the process of preparing for surgery and during laser surgery (drops with 10% concentration).

In addition, the drug is often used for differential diagnosis of superficial and deep eye injection. Eye drops Irifrin bk used in the treatment of true and false myopia (myopia).

Contraindications

  • hyperthyroidism;
  • hepatic porphyria;
  • children's age to 12 years (eye drops 10%);
  • premature (eye drops 2.5%);
  • narrow-angle or narrow-angle glaucoma;
  • aneurysm of arteries (eye drops 10%);
  • hypersensitivity to the drug;
  • congenital deficiency of glucose-6-phosphate dehydrogenase;
  • patients of advanced age with serious disorders from the cardiovascular system and cerebral circulation;
  • for additional dilatation of the pupil during surgical operations in patients with impaired integrity of the eyeball, as well as in violation of tear production.

With care, it is necessary to use Irifrin in patients with diabetes because of the risk of developing an increase in blood pressure associated with with a violation of autonomic regulation, as well as in elderly patients - because of the increased risk of reactive miosis.

Caution should be applied to Irifrin simultaneously with MAO inhibitors, and also within 21 days after discontinuation of their administration.

Application in pregnancy and lactation

According to the instructions for use, the use of the drug during pregnancy and breastfeeding is possible after careful examination by the doctor of the individual characteristics of the patient. If the potential benefit to the mother exceeds the possible risk to the fetus or the baby, the medication can be used, but under strict doctor control.

Instructions for use

The instructions for use indicate that Irifrin in the form of eye drops is applied topically.

When carrying out ophthalmoscopy, single instillations of 2.5% solution. To create mydriasis, it is enough to inject , % irifrin into the conjunctival sac 1 drop. The maximal mydriasis is noted after 15-30 minutes and persists for 1-3 hours. If it is necessary to maintain mydriasis for a long time after 1 hour, repeated instillation is performed.

In children older than 12 years and adults with insufficient dilatation of the pupil, as well as in patients with a rigid iris, a 10% solution is administered in the same dosage for diagnostic dilatation of the pupil.

When performing diagnostic procedures, a single instillation of 2.5% solution is used:

  1. Provocative test in patients with a narrow angle profile of the anterior chamber and suspected of closed-angle glaucoma. The results of the test are considered positive in cases where the difference between intraocular pressure values before application of Irifrin and after pupil dilatation fluctuates within 3-5 mm Hg;
  2. Differential diagnosis of the type of eyeball injection. With narrowing of the vessels of the eyeball 5 minutes after instillation, the injection is classified as superficial, while maintaining redness of the eyes carefully examine the patient for the presence of scleritis or iridocyclitis, as this indicates the expansion of more deeply lying vessels.

In iridocyclites 2.5 or 10% solution is used to prevent the development and rupture of the already formed posterior synechiae, as well as to reduce exudation in the anterior chamber of the eye. With these indications, one drop of Irifrin is buried in the conjunctival sac of the affected eye (eyes) 2-3 times a day.

Due to the vasoconstrictive effect of phenylephrine in glaucoma-cyclical crises, intraocular pressure decreases. This effect is more pronounced when applying 10% Irifrin. To stop glaucoma-cyclical crises, the drug should be digested 2-3 times a day.

When preparing a patient for surgical interventions for 30-60 minutes before the operation to achieve mydriasis, a single instillation of 10% solution is carried out. Re-instillation of the drug is not allowed after opening the shells of the eyeball.

Side effects

Patients' comments and instructions for use among ophthalmic side effects are called:

  1. Common: burning with instillation, pain in the eye, discomfort, conjunctivitis, keratitis;
  2. Moderately common: increased intraocular pressure, periorbital edema, blockade of the anterior chamber, allergic symptoms, reactive hyperemia;
  3. Rarely: reactive miosis, the presence of pigmental leaf elements in the anterior chamber of the eye.

Manifestations from the systemic organs may be contact dermatitis (infrequently).

Among the side effects of the cardiovascular system, patients' reviews are called tachycardia and heart palpitations, arrhythmia, and blood pressure.

Instruction to rare occurrences refers to the occlusion of the coronary arteries, pulmonary embolism.

Overdose

When an overdose of the drug, the listed side effects are observed, but more pronounced. In such cases - immediately stop using the medicine, rinse the conjunctiva eye, seek qualified medical help.

For relief of negative overdose: anxiety, nervousness, sweating, dizziness, vomiting, increase Heart rate, superficial and weak breathing, alpha-adrenoreceptor blockers are prescribed - phentolamine 5-10 mg intravenously.

special instructions

Due to the fact that the drug causes hypoxia of the conjunctiva, it should be used with caution in patients with sickle cell disease anemia, with the use of contact lenses, after surgical interventions (reduction of healing).

Exceeding the recommended dose when using eye drops 2.5% in patients with trauma, eye disease or its appendages, in postoperative period or with reduced tear production (anesthesia) can lead to an increase in the absorption of phenylephrine and the development of systemic side effects.

Drug Interactions

  1. Irifrin enhances the depressant effect on CCC in inhalation anesthesia.
  2. Atropine enhances myrrhatic action of Irifrin with a parallel development of increased heart rate.
  3. The vasopressor effects of phenylephrine increase propranolol, reserpine, guanethidine, m-holinoblockers and methyldopa.
  4. Local anesthetics increase the duration of mydriasis.
  5. Beta-adrenoblockers, when combined with irifrin, weaken their hypotensive effect, which is dangerous due to the development of hypertensive crisis.
  6. Inhibitors of monoamine oxidase and tricyclic antidepressants on the background of the use of Irifrin increase the risk of developing uncontrolled arterial hypertension. The appearance of this effect can be avoided without prescribing Irifrin for 3 weeks after the abolition of MAO inhibitors.

Reviews

We picked up some feedback from people about the preparation of Irifrin:

  1. Valentina. I'm using Irifrin for the first time - I made a course for myself a few years ago, but eventually I started seeing and had to buy drops again. Then I took the drug in a vial, but now I saw that there was a new form - a large package with small flacons "at once." I thought that at one time, I decided to try a new package and began to drip. A week later read the reviews and realized that all this time in vain was translating the medicine. it turns out that one such bottle is not enough for one time, but for two: in one package, eight drops, and they can be divided as follows: on day 1, 2 drops in both eyes, and on day 2, 2 drops in both eyes. The medicine will not spoil because sterility is stored just a day. It turns out a savings of two times)).
  2. Albina. Recently noticed that my eyesight is falling on one eye. After the visit to the oculist, it became clear that my sensations are correct and one eye sees normal, while on the other the vision fell by one. The doctor advised not to buy glasses yet and try to be treated with drops of Irifrin. They are expensive, but what you will not do for your own health. According to the doctor's recommendation, he instilled the drug in the eyes every night, before going to bed. Sensations while not very pleasant, the eyes immediately began to pinch and burn, so that probably the most part of the medication came back together with tears. As a result, I did not feel any effect, so the treatment stopped.
  3. Marina. They removed the thread, fatigue and redness. "Side effect" was a slight improvement in vision, which is also nice) The scheme of the procedure is simple: you drip once a day at night. Side effects - a little pinches and the pupil expands (because of this you can see worse near). In the morning, the eyes are fresh and rested! One bottle is enough for 3-4 weeks.
