Symptoms and forms

The patient has yellow coloring of the skin, mucous membranes and other tissues and body fluids due to an increase in the bilirubin content. The amount of bilirubin in the blood serum in the determination by the van den Berg method is normally 0.2-1 mg%.Distinct icteric staining of the skin and mucous membranes is observed with an increase in the level of bilirubin above 3 mg%, the initial signs of it can be detected with an increase in bilirubin up to 1.2-1.5 mg / o.

Depending on the mechanism of occurrence of jaundice, the forms mechanical , parenchymal , hemolytic and the so-called benign are isolated.

Mechanical jaundice

Occurs when there is an outflow of bile in the biliary tract. In this case, extrahepatic occlusion of the bile duct with a stone, a tumor of the head of the pancreas or bile ducts is more often observed. Less often there is intrahepatic occlusion of small bile ducts, especially with cholangitis, biliary cirrhosis, and also in some cases of viral hepatitis. Jaundice in all these cases can be very pronounced, usually accompanied by itching, bradycardia, which depends on the delay of bile acids. In the serum, an increase in the content of bilirubin, cholesterol is determined. The feces are discolored. Urine is usually dark due to an increase in bilirubin.

Parenchymal jaundice

Occurs when a liver cell is damaged that does not provide enough bilirubin release from the blood to the bile duct. It is observed in viral, toxic hepatitis, liver cirrhosis, etc. In some cases, parenchymal jaundice is combined with mechanical intrahepatic jaundice. With parenchymal jaundice, the serum bilirubin usually increases, and usually not so much as with mechanical jaundice;there are shifts in sediment samples( positive molding, etc.), cholesterol tends to decrease due to a violation of its synthesis by liver cells. Cal may be discolored, but usually only for a short time. Complete disappearance of sterco-bilin in the feces and urobilinogen in the urine is not observed.

Hemolitic icterus

Occurs when the hemolysis is sufficiently expressed( see Hemolytic anemia) due to increased erythrocyte destruction and increased bilirubin formation. Jaundice is usually not expressed clearly, the color of urine is not changed. Benign jaundice

Due to either a violation of the seizure of bilirubin by liver cells from the blood( Gilbert's syndrome), or a violation of excretion, bilirubin in the bile ducts( Dubin-Johnson syndrome). These disorders are associated with congenital defects of some enzymes of liver cells. In Gilbert's syndrome, jaundice can be quite significant( bilirubin can be raised to 5 mg%), however, with hepatic biopsy of the liver, changes in it are not detected, and this anomaly has no significant clinical significance. The Dubin-Johnson syndrome is observed less often, with it in the liver there is a large deposition of melanin. Treatment of jaundice in each case is carried out depending on the nature of the underlying disease.

As you have already understood from the foregoing, the concept of "jaundice" includes at least four forms of the disease, but in this article we consider " parenchymal " the form of jaundice associated with hepatitis B infection of the hepatitis A virus.

Source of infection

The source of infection in viral hepatitis is a sick person. Infection occurs most often in a domestic setting when the virus enters the mouth. Transmission of the virus from a person to a person occurs through dirty hands, infected water and food, as well as household items. Viral carriers can be flies. In hospital conditions, a person can become infected with Botkin's disease through badly boiled syringes and needles.

People of all ages are ill with epidemic hepatitis, more often children of preschool and school age.

Treatment of jaundice: general recommendations of

Early hospitalization of the patient and bed rest are mandatory.

The patient's diet should contain many vitamins( group B, C, PP, pyridoxine, etc.).The amount of carbohydrates in food should be slightly increased, and fat - limited. Food should be high-calorie( about 3000 calories), rich carbohydrates( 300-500 g) and proteins( 60-100 g).

With good tolerability, fats should not be restricted.

  2. Aminopeptide ( subcutaneously or in the form of drop enemas).

Corticosteroids ( Cortisone, Dexamethasone, Prednisolone, Hydrocortisone). The dose of Prednisolone is 20-40 mg per day( with prolonged therapy 10-20 mg).Dosage reduction is gradual for two weeks and only if overall well-being improves, bilirubin drops to 2 to 3 mg% and positive dynamics of sediment samples.
  8. Potassium chloride or potassium acetate( 2-5 g per day). With the development of edema - diuretic.

Antibiotics and sulfonamide preparations are indicated only in the presence of concomitant inflammatory diseases.
